Advance Directive Resources
The MyCare Advance Directive, which is the Advance Directive we work with the most at Get It Done SB!, was created for uniformity among Santa Barbara residents. However, there are also different Advance Directives for different needs, and it is important to consider the variety of options you have for Advance Care Planning.
The Coalition for Compassionate Care of California (CCCC) provides many tools and resources to help providers have more meaningful conversations about their patients' preferences, values and goals for their medical care. CCCC resources may also help individuals complete an advance directive such as those below.
Dementia Directive: The Dementia Directive is a simple and effective way to communicate your wishes if you were to develop dementia. A Dementia Directive can be used as a stand-alone document to communicate one’s wishes. It can also be included as a supplement, attached to someone’s standard advance directive form.
PREPARE: Going through the PREPARE website and the 5 steps is a great way for someone to learn about what is important in their life and their health care. The website walks people step-by-step through important information, video stories, and questions tailored just for them and helps them complete the PREPARE Advance Directive.
California Advance Healthcare Directive: The California Advance Healthcare Directive helps individuals select a medical decision maker and choose the types of healthcare they want to receive including life support treatment and palliative care.
